As a service to the community and so that those in need may benefit from its classes, The Foundation for Family Values (FOFV) offers a sliding scale fee.

Please determine your family income. To do so, please add together:

• the current annual wages of each wage earner in the household
• any child support money you receive, expressed as an annual number
• any other income you receive, expressed as an annual number

• $0-$20,000
• $20,001-$25,000
• $25,001-$30,000
• $30,001-$35,000
• $35,001-$40,000
• $40,001-$45,000

By requesting a sliding scale discount and indicating your family income above, you are making a financial declaration that the income stated is completely true and that, upon request by FOFV, you will provide proof of family income via copies of your most recent 1040 tax forms and/or any other reasonable forms of proof that FOFV may request.
